The free Tribute Video Worksheet is a practical guide for gathering the essential ingredients needed to create a meaningful video celebrating the life of someone who has died or is nearing the end of life.

It walks you through collecting photographs from childhood through recent years; selecting two to five meaningful songs; gathering key biographical details such as family, education, career, relationships, accomplishments and places lived; capturing the personality behind the facts through favorite memories, stories, interests and the qualities that made the person unique; and identifying the people, places and occasions shown in important photographs.

The checklist helps ensure that the most important photos, music, life details and personal memories have been gathered as you plan your Tribute Video.
